About Us

PACE (Program of All-inclusive Care for the Elderly) is a unique program of long term care that allows elderly adults to REMAIN IN THEIR HOME. We work with you and your loved ones to provide the support at home, medical treatment, and social interaction needed to remain independent.

The key to YOUR success is OUR care team. Our team of physicians, nurses, pharmacists, social workers, physical and occupational therapists and nutritionist get to know you and your needs. Thus we are able to coordinate the proper medical treatment and private services needed for you to remain safely in the home and community you love. Our Medical Director, Dr. Keith VanOosterhout, is the only Geriatrician in our area. Dr. VanOosterhout specializes in the treatment and monitoring of adults as they age. With the help of the entire team we can diagnose and treat many medical issues before they become major health conditions.

Our Day Center is the hub of all activity for PACE of Southwest Michigan. Most of our participants come to the day center a few days a week and are transported to and from the center on one of our PACE buses. While here they are able to see the doctor, attend physical or occupational therapy, have a hot meal, and participate in social or recreational activities.

PACE also encourages caregivers to remain involved in the care and treatment process by providing frequent communication, caregiver classes, and other supportive services.
