We are a skilled home health care agency, proudly accredited by the Accreditation Commission for Health Care (ACHC) and certified by the Centers for Medicare & Medicaid Services (CMS) providing home care services (skilled nursing, physical therapy, occupational therapy, speech therapy, medical social workers, and home health aides) to qualifying patients located in Berrien, Cass, Saint Joseph, Van Buren, Allegan, Ottawa, Kent, and Kalamazoo counties of Michigan, as well as St. Joseph, Elkhart, La Porte, Porter, Starke, Kosciusko, La Grange and Marshall Counties in Indiana.
